In today’s healthcare system, patients often feel lost in a maze of portals, scattered lab results, and confusing medical jargon. Current health record portals are not helping much either – according to a study published in 2024, roughly 75% of insured adult patients did not use a healthcare portal at all in the previous year, even when offered one.
Remembering when symptoms started or which doctor said what can feel impossible. That frustration led Raj Subramonian, Founder and CEO of Mediphant, to build an AI healthcare companion designed to put patients back in control.
“Seeing my father carry thick and heavy folders of lab reports everywhere during his chronic illness made me realize how broken the system is,” Subramonian recalls. “What if patients never had to repeat their story from scratch? What if all their health data lived in one place – private and intelligent?”

A Patient-First Solution
Mediphant’s mission is clarity. “Healthcare is fragmented, and patients pay the price,” Subramonian says. “With Mediphant, your labs, records, symptoms, and notes live in one secure vault you can actually use.”
The platform’s mascot, Elly the elephant, embodies memory and trust – a friendly guide that makes privacy and health data less intimidating.
Trust and Differentiation
Health data tools aren’t new, but Mediphant combines three elements rarely found together: strong privacy safeguards, AI that explains results in plain language, and a design focused entirely on patients.
From day one, the platform has been built around end-to-end encryption, HIPAA compliance standards, and a transparent, privacy-first philosophy. Unlike many apps tied to insurers or advertisers, Mediphant’s model is simple: it never sells data and never shows ads.
Making Health Data Understandable
A core strength of the platform is helping patients interpret their information. Mediphant translates labs into everyday language, tracks symptoms over time, and prepares summaries patients can bring to their next appointment.
For example, a 78-year-old cancer survivor recently uploaded two lab reports – a biochemistry panel and a hematology report – into Mediphant. The platform immediately flagged markers consistent with Stage 3a chronic kidney disease, a detail that had not been highlighted to her earlier. And it was entirely based on the metrics in the reports.
“Patients deserve to know not just what the AI says, but why it says it,” emphasizes Karthik Nataraj, co-founder and Chief Product Officer.
In another case, a man brought his mother to an emergency room that didn’t use Epic. Each doctor asked for her medical history, and he was able to provide it seamlessly from her Mediphant vault. He later shared that Mediphant not only kept her past data accessible but also helped him interpret the new labs and imaging tests ordered during her stay.
“Imagine showing up to your doctor with a clear AI-prepared summary of your history,” Subramonian says.
For those managing chronic illness, features like symptom tracking and personal notes ensure details aren’t lost. “When you forget details, you lose part of your story,” Subramonian adds. “Mediphant remembers you for you, so your care is more connected.”
Looking Ahead
Since launch, Mediphant has drawn attention as a tool that reduces anxiety and increases confidence at the doctor’s office. This is not surprising, since a survey found that 76% of Americans leave their doctor’s office feeling confused about their health information.
“Our vision is to enable patients to use AI not as a wall of complexity, but as a bridge to understanding,” Subramonian says.
With Elly leading the way, Mediphant isn’t just another health app—it’s a step toward giving patients back their voice in a system that too often overlooks it.
